Application for exemption of fees – financial hardship (family law)

This form is used to apply for an exemption of paying fees if payment of a fee will cause you financial hardship and you are not eligible for a general exemption. This form cannot be used for divorce applications.

Application – contempt

This form is used to apply for a person to be punished for contempt of court. An Affidavit must be filed at the same time stating the facts relied on in support of the application

Affidavit of service

This form must be completed, once service is effected, by the person who served the documents. The party who filed the document must then file the Affidavit of Service with the Court to prove service.

Affidavit proof of signature (non-divorce)

This form is used when proof of signature is required to confirm service in a family law proceedings (other than divorce). The signed Acknowledgement of service must be attached to this form.

Affidavit – non-filing of family dispute resolution certificate

If you are applying for interlocutory orders, that is, orders to cover your case until a final hearing can take place, then you do not need to complete this affidavit. You will have to complete an Application in a proceeding and an affidavit in support.
