Filing with the Court
An Application for Consent Orders should be filed electronically on the Commonwealth Courts Portal by uploading the form or if you are unable to eFile, by filing at a court registry.
NOTE: You must also provide an identical unsigned Word document (.docx) version of the proposed orders. The document cannot be ‘locked’ for editing or have any tracked changes, images or macro functions.
