    • Notice of Address for Service

      This form is used to inform the Court and the other parties in your case of the address at which court documents and notices can be served on you. You must file this form if your contact address (address for service) has changed.
    • Notice of Discontinuance

      This form is used if you are no longer seeking all or some of the orders previously sought by you. Filing this notice does not prevent any other party from continuing with their case and obtaining orders including for costs.
    • Parenting Questionnaire

      Unless you are required to file an affidavit, this form is filed at the same time as an Initiating Application (Family Law) or Response to Initiating Application (Family Law) seeking parenting orders.
    • Application – contravention

      You should complete this form if you want to seek an order from the Court imposing a punishment or another consequence on a person for the breach of a Court order. You cannot eFile this type of application.
    • Indigenous Family Liaison Officers

      This fact sheet provides information about the role of Indigenous Family Liaison Officers (IFLOs) in the Federal Circuit and Family Court of Australia (the Courts).
    • Experts conference (prescribed brochure)

      This brochure sets out guidelines to assist people to comply with Part 7.1 of the Family Law Rules (expert evidence). It should be read in conjunction with those Rules.
    • Financial Questionnaire

      Unless you are required to file an affidavit, this form is filed at the same time as an Initiating Application (Family Law) or Response to Initiating Application (Family Law) seeking financial or property orders.
    • Affidavit of service by post (divorce)

      This form is used by a person filing a sole application for divorce who is serving their spouse with the divorce application by post.
    • Pre-Action Procedures: What to do before you file your Family Law application

      This checklist takes you through the steps or Pre-Action Procedures you need to complete before you do so. These steps will ask you to try and resolve issues with your partner using Dispute Resolution, if it is safe to do so.

      With the Pilot now well underway, the Court calls upon the profession to familiarise themselves with this important project and to communicate the process and benefits with their clients.
