    • Affidavit – non-filing of family dispute resolution certificate

      If you are applying for interlocutory orders, that is, orders to cover your case until a final hearing can take place, then you do not need to complete this affidavit. You will have to complete an Application in a proceeding and an affidavit in support.

      The Courts and Tribunal are committed to ensuring the security and integrity of our systems and data. In alignment with the Australian Information Security Manual (ISM), we recognise the valuable role that security researchers and general public play in identifying and reporting vulnerabilities. This Vulnerability Disclosure Program (VDP) outlines the permitted activities a researcher can perform, the process for reporting potential security vulnerabilities, and the of the response Courts and Tribunal.
    • PPP Financial Summary (form)

      This form is to be used in a Priority Property Pool Case (PPP) once the Court makes a declaration or notation that the case is designated as a PPP case.
    • Originating Application for an order of Review under ADJR

      This form is used when a person wants the Court to review an administrative decision or conduct under the Administrative Decisions (Judicial Review) Act 1977.
    • Annexure to proposed consent parenting orders (current case)

      Each party to an application for parenting orders by consent must complete this annexure to proposed consent parenting orders form
