    • FCFCOA Information Notice: Pronunciation of names & forms of address

      This Information Notice applies to all proceedings filed in the Federal Circuit and Family Court of Australia (Division 1) and (Division 2) (Courts) in the Courts’ family law, migration and general federal law jurisdictions.
    • Affidavit proof of signature (non-divorce)

      This form is used when proof of signature is required to confirm service in a family law proceedings (other than divorce). The signed Acknowledgement of service must be attached to this form.
    • Affidavit of service

      This form must be completed, once service is effected, by the person who served the documents. The party who filed the document must then file the Affidavit of Service with the Court to prove service.
    • Application for consent orders (do it yourself kit)

      This form is used when you have reached an agreement and want the Court to make legally binding court orders.
