    • Application – contempt

      This form is used to apply for a person to be punished for contempt of court. An Affidavit must be filed at the same time stating the facts relied on in support of the application
    • Affidavit proving signature (divorce)

      If the person who serves an Application for Divorce by hand does not know the person served, the applicant must complete this form.
    • Notice of Appeal (Child Support)

      This form is used when seeking to appeal from a decision of the Administrative Review Tribunal (ART) in a child support first review or second review or from a decision of the Child Support Registrar (CSR) to issue a departure prohibition order.
    • Notice of Withdrawal as Lawyer

      This form is used by lawyers to advise the Court that they no longer act for their client. A lawyer may file a notice of withdrawal without leave of the Court only if, at least 7 days before filing the notice, the lawyer has served a notice of intention to withdraw on the party for whom the lawyer is acting.

    • Proposed Standard Orders - Migration - Final hearing

      This document contains the proposed standard orders that parties may use to seek timetabling orders from the Court for a final hearing of a migration matter.
    • Acknowledgment – information from a family counsellor or family dispute resolution practitioner

      This form is used by the person applying for a Part VII order where it is alleged that there has been abuse of a child by one of the parties to the proceedings or there has been family violence towards the child or a member of the child’s family by one of the parties to the proceedings. This form is not filed but is provided to the Judicial Officer hearing the case when you first appear in Court.
    • Guidelines for reduced fee - divorce and decree of nullity application

      Information about eligibility to apply for a reduction of divorce fee - general
    • Response – Human Rights

      This form is used to respond to an application in the Federal Circuit and Family Court of Australia (Division 2) alleging unlawful discrimination under the Australian Human Rights Commission Act 1986: see rule 28.05 of the Federal Circuit and Family Court of Australia (Division 2) (General Federal Law) Rules 2025 (the Rules).

  • Application – contempt
