      An affidavit is a written statement where the contents are sworn or affirmed to be true. An affidavit should set out facts, not opinion.
    • Revoked Practice Directions

      Family Court of Australia and Federal Circuit Court of Australia Joint Practice Direction: JPD1 of 2020 – Core principles in the Case Management of Family Law Matters
    • Financial Statement Kit (do it yourself kit)

      This form is used if you are a party to a financial or property proceeding including: Financial or property settlement, Maintenance, Child Support; and Financial enforcement
    • Request to attend by electronic communication

      This form is used to seek permission to attend by telephone or video link.
    • Response to initiating application (do it yourself kit)

      Step-by-step kit to respond to a family law initiating application.
    • Undertaking as to disclosure

      This form is used to acknowledge your duty of disclosure and to undertake that you have complied and will continue to comply with that duty.
    • Application in a proceeding

      This form is used if a party to current proceedings wishes to seek further interlocutory or procedural orders. An Affidavit must be filed at the same time stating the facts relied on in support of the orders sought in the Application in a Proceeding.
    • Applying for a decree of nullity

      This fact sheet is for people who need information about applying to the Federal Circuit and Family Court of Australia (the Court) for an order that a marriage is a nullity, described in the Family Law Act 1975 as a ‘decree of nullity’.
    • Request for file inspection (Family law only)

      This form is used by a party or their lawyer or an Independent Children’s Lawyer to request access to view a file if they are unable to view the documents on the file via the Commonwealth Courts Portal.
