    • Superannuation Information Request (Online form)

      This form is to be used by a party to current property settlement proceedings in the Federal Circuit and Family Court of Australia to request superannuation information about their spouse, former spouse or former de facto partner.
    • Affidavit translation of marriage certificate

      If a marriage certificate is not in English it must be translated. The person who translates the certificate must complete an Affidavit Translation of Marriage Certificate and: attach a copy of the marriage certificate; attach the translation of the marriage certificate; set out their qualifications to make the translation.
    • Bankruptcy - Application for summons to examine relevant person or examinable person

      This form is used when making an application for an examinable person in relation to a relevant person to be summoned for examination under section 81 of the Bankruptcy Act 1966.
    • Affidavit of Debt

      This is an affidavit modified for use pursuant to Rule 4.06(4) of the Federal Circuit and Family Court of Australia (Division 2) (Bankruptcy) Rules 2021 – An affidavit deposing that the debt is still owing.
    • Notice of Intervention by Person Entitled to Intervene

      This form is used if you are entitled to intervene. An affidavit must be filed with this form. If you need permission to intervene you must file an Application in a Proceeding (see Rule 3.04)
    • Statement of financial circumstances - Corporation

      Use this form to provide information about the financial circumstances of a corporation in support of an application to defer payment of court fees on the grounds that, considering those circumstances, it would be oppressive or otherwise unreasonable to require payment of the fee at the time it is required under the Regulation.
