    • Confidential Case Outline document (Dispute Resolution)

      This document is to be provided to the Court via email or private mediator on a ‘without prejudice’ basis prior to dispute resolution.
    • Certificate of non-compliance with subpoena (New Zealand)

      This form is used to apply for a Certificate of non-compliance with subpoena if the person named in the subpoena (which was served in New Zealand) fails to comply with the subpoena.
    • Application for Review

      This form is used to apply to the Court for a review of the exercise of a power by a Registrar in proceedings other than appeal proceedings. Use an Application in an Appeal to make application for review of an exercise of power by a Registrar in appeal proceedings. Use a Form B3 – Application for Review in bankruptcy proceedings.
    • Notice of Request to Inspect

      This form is used by the person who issued the subpoena to seek permission to inspect a subpoena. 
    • Application for sealed copy of orders

      If you require a sealed copy of orders from a file that are not available from the Commonwealth Courts Portal and you were a party to the proceedings you should complete the Application for sealed copy of orders. It is important to include the full names of the parties. Most orders made after July 2017 are available to download from the Commonwealth Courts Portal at no cost.
    • Family Law Practice Direction: Corporations Act proceedings

      This Practice Direction applies to proceedings in the Federal Circuit and Family Court of Australia to which any of the following legislation applies: Corporations Act 2001; Australian Securities and Investments Commission Act 2001; or Corporations (Aboriginal and Torres Strait Islander) Act 2006.
    • Family Law Practice Direction: Evatt list

      This Practice Direction sets out the Evatt List eligibility for proceedings filed in the Court and sets out the case management of matters in that list.
    • Notice of child abuse, family violence or risk

      This is a mandatory form that must be filed with any Initiating Application, Response to Initiating Application or Application for Consent Orders seeking parenting orders. This form is not to be filed if the orders sought relate to property or financial matters only.
