    • Financial Statement Kit (do it yourself kit)

      This form is used if you are a party to a financial or property proceeding including: Financial or property settlement, Maintenance, Child Support; and Financial enforcement
    • Application in a proceeding

      This form is used if a party to current proceedings wishes to seek further interlocutory or procedural orders. An Affidavit must be filed at the same time stating the facts relied on in support of the orders sought in the Application in a Proceeding.
    • Request for file inspection (Family law only)

      This form is used by a party or their lawyer or an Independent Children’s Lawyer to request access to view a file if they are unable to view the documents on the file via the Commonwealth Courts Portal. 
    • Initiating Application Kit (do it yourself kit)

      This form is used to start a family law application seeking final orders. Interlocutory orders can only be sought in this form if you are seeking final orders as well.
    • Notice of Address for Service

      This form is used to inform the Court and the other parties in your case of the address at which court documents and notices can be served on you. You must file this form if your contact address (address for service) has changed.
    • Child Impact Report - FAQs

      This fact sheet provides information for parties who have had a Child Impact Report ordered by the Court.
    • Parenting Questionnaire

      Unless you are required to file an affidavit, this form is filed at the same time as an Initiating Application (Family Law) or Response to Initiating Application (Family Law) seeking parenting orders.
    • Affidavit for eFiling application (Divorce)

      The Affidavit for eFiling (Divorce) forms part of the Application for Divorce and must be signed by the applicant or applicants for a divorce to proceed
    • Balance sheet

      This form is not a prescribed form, but you may be directed to prepare one in your case. A balance sheet is used to list assets and liabilities held by parties. The balance sheet also includes add backs, financial resources and superannuation. In order to prepare a Balance Sheet, you will need to itemise, describe and provide your value for each of your assets and liabilities.
