Affidavit of service by hand (divorce)

This form is used by a person filing a sole application for divorce who is arranging service of the divorce application on their spouse by hand.

You cannot serve the documents on your spouse yourself.

You must arrange for a person over 18 years of age (the server) to serve the documents on your spouse (the server). The server can be a family member, friend or professional process server.

In completing a Court form, you must*

Complete the form by typing or hand printing in ink.
