This form is used to apply for a final Commonwealth workplace protection order (CWPO) or a consent order. An application for a final order may include an application for an interim order.
The evidence supporting an application for a CWPO of any kind must be provided by affidavit, statement of claim or points of claim filed with the application (see rule 8.04 of the General Federal Law Rules).

Filing with the court
Once complete, you need to file the application with the Court. It must be filed electronically using eLodgment, unless it is not reasonably practicable to do so.
